USB Cable Separates From Connector After Only Couple Uses iPhone 3G

I have an iPhone 3G and found that the included USB cables covering separated from the USB connector after just a couple of times moving the cable. How can I go about getting this replaced? I have another cable I usually use, so I dont mind sending it in. I went to submit a request online, but none of the options really fit. Thanks!
