Restored Phone-still displaying old Apple ID iPhone

I have a new iPhone 4 for verizon. I wanted to give my old iPhone 4 for AT&T to a friend. So I wiped it clean using the restore option. Back to factory settings. Or so you would think. True to iTunes form, it did something that makes no sense. Everything is back to factory. Except when she goes into the App Store it displays my Apple ID, and asks for my password.
This is a phone I have not even used on a network in over 6 months.
How do I undo this so she can enter her own information. Common sense / logic would state restoring the phone to factory settings would also reset this. But iTunes doesnt major in doing logical, intuitive things.

On your old iPhone 4, Settings>General>Reset>Erase All Content & Settings. That should take but a minute or so & when finished the phone should look just like it did when first taken out of the box when all your data gone including your Apple ID.
