Out of Warranty Service for iPhone 3G-OOW

Hi,
Im thinking about sending a very bad shape but fully functional iPhone 3G for Apple OOW service.
The screen is broken, but if for some reason they find it unrepairable or really bad, will they refund the 200$ fee?
And if they take it, will they replace it for the exact same model? or upgrade it to a 3GS?

The iPhone is exchanged, not repaired. If Apple decides your iPhone is not acceptable under their OOW service, you wont be charged $199 plus tax. You will be charged $199 plus tax if your iPhone is exchanged under Apples OOW.
If accepted, your iPhone will be exchanged with the exact same model.
